Doctors have long used bowel habits as clues to identify digestive problems, infections, and even serious illnesses.

A healthy gut has become one of the biggest focuses in modern medicine.

Researchers continue to uncover new ways the microbiome influences everything from immunity to brain function.

Now, scientists believe your stool may reveal something even more remarkable.

A new study suggests that a single sample could help predict whether a critically ill patient is at high risk of dying within the next month.

While the findings are still in the early stages, researchers believe the discovery could eventually become a valuable tool for hospitals treating the sickest patients.

Scientists create a new way to measure gut health

The research was carried out by scientists from the University of Chicago and the University of Amsterdam, led by Alexander de Porto.

Rather than examining stool for signs of infection or disease, the researchers focused on chemicals produced inside the gut.

These compounds, known as fecal metabolites, are created through interactions between bacteria living in the digestive system and the food we eat.

Using these metabolites, the team developed a scoring system called the Metabolic Dysbiosis Score (MDS).

The score measures microbial imbalance inside the gut, a condition known as dysbiosis.

Instead of simply identifying whether someone’s microbiome is healthy or unhealthy, the MDS analyzes 13 specific metabolites to determine how severely the gut ecosystem has been disrupted.

Researchers hope the score will eventually help doctors identify critically ill patients who face the greatest risk of deterioration.

This could help medical teams to make faster and more informed treatment decisions.

The researchers didn’t create the test for healthy members of the public checking their wellbeing at home. Instead, they designed it specifically for patients already receiving intensive medical care.

Why your gut could reveal more than digestive problems

Scientists have increasingly linked gut health to overall wellbeing during the past decade.

Your gut microbiome contains trillions of bacteria, fungi, viruses, and other microorganisms that help digest food, regulate the immune system, produce essential nutrients, and even communicate with the brain.

A balanced microbiome supports many of the body’s vital functions, while an unhealthy one can contribute to a wide range of medical conditions.

Previous research has shown that people with greater microbiome diversity often experience better long-term health, while lower diversity has been associated with chronic illness and poorer outcomes.

When the microbiome becomes unbalanced, dysbiosis develops.

Many people experience relatively mild symptoms, including bloating, excessive gas, stomach discomfort, or changes in bowel habits such as diarrhea or constipation.

However, more severe microbial imbalance has also been linked to conditions including Crohn’s disease, bacterial infections, and other inflammatory bowel diseases.

Researchers also believe the gut-brain connection means these imbalances may affect much more than digestion.

Signals constantly travel between the digestive system and the nervous system.

This means disruptions in gut bacteria could influence inflammation throughout the body and potentially impact neurological health.

The new research suggests that when the microbiome becomes severely disrupted during critical illness, those changes may reflect how well the body can survive.

Rather than simply indicating digestive health, stool samples may provide doctors with another window into a patient’s overall resilience.

The study produced surprisingly accurate results

To test the new scoring system, researchers analyzed stool samples from 196 patients receiving treatment in intensive care units.

The participants had been admitted with severe conditions including respiratory failure and shock, placing them among the sickest hospital patients.

After analyzing each sample, researchers compared the Metabolic Dysbiosis Score with patient outcomes.

The results were striking.

According to the study, the MDS predicted mortality with approximately 84 percent accuracy.

While no medical test can predict outcomes with complete certainty, the researchers believe these results demonstrate that gut-derived metabolites could become an important biomarker for identifying patients facing the greatest danger.

Speaking to Science Alert, Eric Pamer and Bhakti Patel, members of the research team, explained the significance of the findings.

“The findings suggest that fecal metabolic dysbiosis, quantified through the MDS, holds potential as a biomarker to identify critically ill patients at increased risk of mortality.

“This underscores the importance of gut-derived metabolites as independent contributors to host resilience, offering an avenue for precision medicine.”

The researchers believe the test could eventually complement existing tools doctors already use when assessing critically ill patients.

Physicians may one day also examine the condition of the gut microbiome when making treatment decisions.

The biggest limitation is the study’s size.

While 196 patients provided encouraging results, that number is still relatively small by medical research standards.

Before hospitals begin routinely using the Metabolic Dysbiosis Score, scientists need to repeat the study with much larger groups of patients to confirm its reliability across different hospitals, illnesses, and populations.

In other words, researchers simply need to study many more stool samples before they can say with confidence that the test works consistently.

Even so, scientists believe the technology has considerable potential.

Fecal metabolites have already shown promise in other areas of medicine, including identifying liver transplant patients who face higher risks after surgery.

That means the same approach could eventually help doctors monitor patients recovering from transplants, major operations, infections, or other life-threatening illnesses.

For now, the research shouldn’t encourage healthy people to inspect their stool for signs of impending death. The Metabolic Dysbiosis Score was designed specifically for critically ill patients in intensive care, not for routine health checks.

However, the study highlights just how much information the human gut contains.

Scientists continue discovering that the microbiome influences far more than digestion, and future hospital care may rely increasingly on what is happening inside the digestive system.

If larger studies confirm these findings, one day a stool sample could become another routine tool doctors use to identify patients most at risk.
