The former X Factor judge, 64, has been heavily criticised for his ever-changing looks in recent years.
But Cowell’s latest public appearance actually has some fans feeling worried.

Critique over Cowell’s face all started when the Britain’s Got Talent X (formerly Twitter) account uploaded a now-deleted video promoting the series in early 2023.
One person wrote: “Well… I’m torn between my general hatred of Simon Cowell’s TV shows and being alarmed by how he looks these days. I’m going for the latter this morning.”
“Is that Simon Cowell? What’s happened to his face?” another questioned.
A third said: “That doesn’t look like him at all.”
While a fourth asked: “What’s he done to his face?”
Cowell did address this backlash in an interview with The Sun.
He explained: “There was a stage where I might have gone a bit too far. I saw a picture of me from ‘before’ the other day, and didn’t recognise it as me first of all.
“Enough was enough. There is no filler in my face at all now. Zero. There was a phase where everyone was having their faces pumped full of this and the other.”
Cowell also told Page Six: “I reduced it, let’s put it that way. Everything in moderation.”
But it seems Cowell has not stayed true to his word, as things have become even more extreme since then.
This weekend, Cowell appeared on Ant McPartlin and Declan Donnelly’s comedy game show, Saturday Night Takeaway.
And his extreme new look attracted a lot of comments from confused and baffled viewers.
“I was shocked to see Simon Cowell on TV last night. What on earth has he done to his face?” exclaims a viewer on X.
Another asks: “What’s with Simon’s face?”
Cowell’s appearance was so controversial that he even became the subject of jokes from other guests on the show.

During the programme, Cowell was a part of a sketch during which he was pulled over by a fake police officer.
After watching the prank, fellow guest Stephen Merchant joked: “I didn’t like it, I loved it.
“That’s my Simon Cowell impression. It’s not very convincing because my face moved.”
It wasn’t just Cowell’s appearance that had fans concerned, either.
One worried fan writes: “Oh God, putting the face aside, he looks like he could hardly walk. What has happened to Simon Cowell?”
Another agrees: “Simon Cowell is looking rather thin and walking slightly weirdly in my opinion.”
