What will be the first thing you hear if Donald Trump dies in office?

The President of the United States is, obviously, still alive, but with his advancing years and constant concerns regarding his health, the question of what happens if he passes away while in office keeps rearing its head.

Trump, of course, saw two attempts made on his life during his Presidential election campaign trail in 2024. He was shot at during a rally in Pennsylvania and then another would-be assassin was arrested as he planned to shoot Trump while he was playing golf.

Iran have also made public assassination threats towards the President.

Recently, attention has turned towards the President’s health. Trump has appeared to have fallen asleep during several meetings in the last few months.

Severe bruising on his hands has been questioned. These bruises have since been attributed to aspirin use, however, which Trump has addressed in interviews recently.

In October the two-time President underwent a CT scan at Walter Reed Military Medical Center, stating at the time: “I got an MRI. It was perfect. I mean, I gave you the full results. We had an MRI and the machine, you know, the whole thing, and it was perfect.”

While Trump stated that he had an MRI, although it was in fact a CT scan, as per Forbes.

How is Donald Trump’s health?

Given that the US President is the oldest sitting President in America’s history, it is only natural that his health is discussed so regularly, especially when you consider the enormity of the job.

White House Press Secretary Karoline Leavitt did confirm in July 2025 that the two-time President had been experiencing swelling in his legs.

This was later diagnosed as chronic venous insufficiency – a condition which sees the legs struggling to send blood back to the heart due to faulty valves, resulting in a pooling of blood.

Regarding the President appearing to fall asleep or not look fully awake during meetings and events that have been broadcast, Trump has attempted to address these concerns as him blinking or simply resting.

Former White House doctor Jonathan Reiner has raised concerns about Trump’s health.

The doctor, who previously served as a cardiologist for former Vice President Dick Cheney, shared a video on X in the middle of January that he believes suggests Trump should undergo a new medical evaluation.

Reiner also cited a speech Trump had made at the time, highlighting his tone and ‘manic’ pacing.

“I’m seriously concerned about the health of the president,” Reiner commented at the time. “No one should be happy to see the president like this. He looks unwell.”

Earlier this year, Trump’s niece, Mary, the daughter of the President’s older brother Fred Trump Jr., spoke out about her concerns regarding her uncle’s cognitive awareness.

In a video on her YouTube channel titled ‘Trump declines rapidly as behavior raises concerns,’ Mary, a psychologist and regularly vocal critic of her uncle and his presidency, highlighted several moments where the President showed signs of ‘losing it.’

According to his niece, Trump’s ‘mental decline’ seems to be something that has gone under the radar, given his controversial personality.

Mary said: “We’ve all become used to the fact that every time Donald stumbles, rambles, forgets people’s names, wanders off the stage, or just pulls made-up facts out of thin air.

“In other words, lies, people on the right pass it off as if these are just fun little quirks of their dear leader, and not the fact that every single day Donald Trump appears to be losing it.”

What happens when a sitting President dies?

Regarding the immediate process of what happens if a sitting US President dies, the succession plan, as per the Constitution, states that the current Vice President ascends to the Presidency automatically.

In this case, were Trump to die during his second term, JD Vance would become US President.

Article II, Section 1, Clause 6 of the Constitution states: “In Case of the Removal of the President from Office, or of his Death, Resignation, or Inability to discharge the Powers and Duties of the said Office, the Same shall devolve on the Vice President.”

The immediate succession is in place to ensure no potential instability occurs in the aftermath of a Presidential passing.

With the Vice President becoming President in the event of a Presidential death, the first thing anyone would hear if Donald Trump dies in office would be JD Vance taking the oath as he is sworn in as President of the United States.

Vance would then have the power to make any changes he wished to the former President’s cabinet and would select a new Vice President, too.

For the Vice President to assume the Presidency in this manner, if they serve less than two years before the next election, they can serve two more full terms, should they be elected successfully.

However, if their initial term after taking the Presidency is longer than two years, they can only be elected for one further full term.
